Los efectos antiinflamatorios y analgésicos de EPA y DHA se deben a su capacidad para ser convertidos en lípidos bioactivos denominados oxilipinas pro-resolutivas de la inflamación o SPM, capaces de ejecutar mecanismos atenuantes de la inflamación e incluso promover su resolución, con un efecto inhibidor directo sobre la síntesis de TNF-α, IL-1 e IL-2. Este efecto resuelve una de las causas más comunes del dolor crónico: la inflamación. Sin embargo, las oxilipinas sintetizadas a partir de EPA y DHA son también capaces de promover la analgesia al actuar sobre los nervios nociceptivos, induciendo una mayor liberación de péptidos opioides responsables de “cortocircuitar” la transmisión del dolor. Estos efectos no son solo son positivos sobre el dolor e inflamación asociados al ejercicio o las enfermedades inflamatorias y autoinmunes, sino que también contribuyen a la prevención de accidentes cardiovasculares y la reducción de los síntomas asociados al cáncer y su tratamiento, como caquexia, neuropatía o complicaciones post-operatorias.

La relación Ácido Araquidónico:EPA (o AA:EPA) se está aceptando como un predictor específico de riesgo cardíaco y de inflamación crónica en general. Esta relación indica un mejor o peor equilibrio entre “buenos eicosanoides” y “malos eicosanoides” en plasma. Un ratio elevado indica un exceso relativo de AA con respecto al EPA que se corresponde con un estado proinflamatorio, de proagregación plaquetaria y provasoconstricción. Cuando el ratio AA:EPA es elevado, si un proceso inflamatorio se activase en el organismo, la reacción inflamatoria sería anormalmente exagerada. En los pacientes con condiciones inflamatorias o desórdenes neurológicos el AA/EPA puede exceder de 15.
